About Your First Visit

Please bring with you the following:

Any x-rays your dentist has provided you with. We often contact your dentist to mail them. Bring all important dental insurance information, such as benefits booklets and a dental insurance form.

At your first visit you will be required to fill out a medical questionnaire. Please bring a list of all medications you are on and dosages--if this applies.

Dr. Smook will examine you, evaluate radiographs, present a treatment plan discussing with you all aspects of your periodontal or dental implant case at the first vist. All questions will be answered and fees & scheduling will be gone over.
